By Submitting this Form, you understand and agree to the following:

Terms and Conditions

1. I understand that the decision to approve or deny my request to change my host company is at sole the discretion of the of Institute of Global Exchange, my program sponsor.
2. I understand that I must obtain permission from Institute of Global Exchange to change my host company during my Exchange Visitor Program.
3. I understand that I am responsible for proper submission of all required documents by the deadlines and in the manner required by Institute of Global Exchange for Institute of Global Exchange to review my request to change my host company.
4. I understand that leaving my current approved host company without prior approval from Institute of Global Exchange could result in my J-1 visa sponsorship being terminated.
5. I understand that I must receive explicit written approval from Institute of Global Exchange before I may begin training at a new host company.
6. I understand that beginning training at a location not approved by Institute of Global Exchange could result in my visa sponsorship being terminated.
